GAPP

Everlasting Growth is a participating provider in the Georgia Pediatric Program (GAPP). It is Medicaid’s in-home nursing program designed to serve children under the age of 20 years and 11 months with activities of daily living needs such as feeding, bathing, incontinence care, transfers, grooming, dressing, G-tube care, medication administration and more at NO COST to the family.

Children must be receiving physician-ordered services based on a medical necessity determination i.e., medically fragile or complex diagnoses to be eligible for nursing care or personal care. The parents or child's guardian are involved in the development of their child's service plan of treatment along with any service goals.

Eligible members age out of the GAPP program on their 21st birthday.
